We are changing the way we would like patients to contact the practice. As of the 6th September, we would like all patients to complete an e-consultation for urgent and routine appointments.

We ask that on completing the consultation form you add as much information as possible to aid the clinical team to triage, assess and allocate the appropriate appointment in line with each individual patients need.

Our aim is to

· Ensure all contacts with the practice for an appointment will be triaged by the clinical team

· Improve access to same day/urgent services

· Reduce routine appointment waiting times

· Improve waiting times on phones

· Improved signposting to appropriate services

We are aware that not everyone has access to the internet. We kindly ask these patients to contact the practice by telephone and request the help of the healthcare administrator, who will complete the form on your behalf.

We will also provide a screen in practice for you to pop in and complete the form.
